myTMH Patient Portal

Welcome to myTMH, your free, secure, personal gateway into your medical care at Tallahassee Memorial HealthCare. Our online patient portal allows you to view your electronic medical record at your convenience.

Through the portal you can:

  • Access your electronic medical information (for example: results, medications, procedures, diagnoses, consultation notes, discharge summary notes, history and physical exam information, imaging narratives, laboratory report narratives, pathology report narratives, procedure notes, progress notes and other treatment related information from your TMH visit).
  • Send medical information to other healthcare providers.
  • Connect the myTMH Patient Portal to select health management apps.

Register for the myTMH Patient Portal

PatientPortalEnrollGreenIf you are a registered patient at Tallahassee Memorial HealthCare (TMH) and 18 years old or older, you can self-enroll in the myTMH Patient Portal. Enrolling in the Patient Portal is quick and easy. Here are the steps:

  • To enroll, you will be required to enter either your e-mail address or your Medical Record Number (MRN) and the last four digits of your social security number. If you do not know your MRN, please call Medical Records at 850-431-0965 to complete your registration.
  • Once you have your MRN, click on “Register” to begin registration.
  • Complete all the required fields, then click “Submit”.
  • You will be asked to verify your identity and accept the terms of use. Please review the information provided then click “Next”.
  • You will then be redirected to create a personal login with Cerner, the electronic medical record system used by TMH. Enter your email address under the “I Need an Account” heading.
  • Complete the registration information to create an account. Please don’t share your username and password with anyone.
  • You are now in the myTMH Patient Portal. You are now able to check your TMH test results and access your TMH electronic medical records at your convenience.
