Interested in the health of our community?

Tallahassee Memorial HealthCare conducts a community health needs assessment at least once every three years to meet the needs of our community’s health. Our hope is to be a catalyst for change in our community through education, health programs and partnership surrounding health and overall well-being.

In order to best serve our community, please submit your feedback through our 2019 Community Health Needs Assessment.

Our survey can be easily completed digitally or by mail (please see contact information for mailing address). The survey takes approximately 10 minutes from start to finish.

Participate in the 2019 Community Health Survey

We’d love to hear from you. Please complete the Community Health Survey using one of the methods below. This survey is completely anonymous (you remain un-named and unknown to us) and confidential (all information collected is private and only shared as group summary).

The community survey will be available through March 8, 2019 for all residents, 18 years of age and older, living in Gadsden, Jefferson, Leon and Wakulla counties.
