Hospital Transfers and Physician Referrals

The TMH Transfer Center

The TMH Transfer Center is a 24/7 department that works with physicians and other healthcare providers to assist with physician-to-physician consults and patient transfer requests. Every day and around the clock, the Transfer Center works with referring facilities to facilitate rapid, safe transfers for patients with all types of illness or injury from healthcare facilities throughout North Florida, Southeast Alabama and Southwest Georgia.

The Transfer Center works closely with referring facilities to obtain the necessary clinical information, imaging and documentation requested by the receiving physician to facilitate a safe and timely transfer. Our goal is to streamline the referral and acceptance process, making it faster, more efficient and convenient through improved communication.

Transfer Care Services

With one call, physicians and other healthcare providers can request a physician-to-physician consult or initiate a patient transfer. A Transfer Center Specialist remains on the line throughout the process to coordinate communication and assist with all transfer logistics.

Transfer services include:

• Physician-to-physician consults
• Inpatient-to-inpatient transfers
• Emergency department-to-emergency department transfers
• Adult and pediatric patient transfers

To Request a Consult or Transfer

• Call 850-431-4196.
• Have the patient's information available, including:
  • Patient name
  • Condition and diagnosis
  • Vital signs
  • Reason for transfer or consultation
  • Referring provider information
• The Transfer Center Specialist will obtain all necessary clinical information, imaging and documentation needed for physician review.
• Once all required information has been received, the Transfer Center Specialist will connect the referring and receiving physicians for a peer-to-peer discussion.
• If the receiving physician accepts the patient and TMH can provide the necessary level of care, the Transfer Center Specialist will coordinate with Nursing Supervisors to assign an appropriate bed and facilitate the transfer
