You are now accessing the MyChart medical information for your loved one who has either agreed to give you this access or for whom you have a legal responsibility. By accepting this proxy access disclaimer, you are verifying that you have the right to access this information, granted to you by the patient and Tallahassee Memorial HealthCare, Inc. (“TMH”).  If you feel that you've received this access in error, please contact 850-431-6019.

3. When your legal authority to act on behalf of the patient has been inactived, revoked, terminated or expired, you will immediately notify TMH in writing of the change in authority.

1. If a request to terminate the access is submitted; or
2. If the legal relationship between myself and the patient changes and no longer permits me the right to view his/her medical records; or
3. If TMH in its reasonable discretion determines that cause exists to terminate access.
