Heart Failure Care


From the hospital to your day-to-day life, our comprehensive Heart Failure Program will give you the tools you need to manage congestive heart failure and improve your long-term health.

What is heart failure?

Congestive heart failure is a condition in which your heart can’t pump enough blood to meet your body’s needs. In some cases, the heart can’t fill with enough blood, and in others it’s not strong enough to pump blood throughout the body. Some people have both problems.

The term “heart failure” doesn’t mean your heart has stopped or is about to stop working. However, heart failure is a serious and often chronic (long-term) condition that requires medical care. By taking an active role in your care and working closely with your healthcare team, you can live longer and more comfortably.

About Our Program

Heart failure can be challenging. It’s also one of the leading causes of hospitalization in the US. Our Heart Failure Program will provide you resources, education and services to improve your health and meet your individual goals.

Through the program, we strive to:

  • Relieve symptoms and improve your quality of life
  • Slow disease progression
  • Reduce the need for emergency room visits and hospitalization
  • Help you live longer

Our Heart Failure Program team supports patients with the following:

  • Medications: We’ll help you get the medications need and educate you on how and when to take them.
  • Nutrition: You’ll have a dedicated dietician to educate you on which foods to eat and which foods to avoid.
  • Clinic Access: Our program offers you easy access to the heart failure team as often as needed, with a goal of helping you avoid having to go to the emergency room or hospital.
  • Overall Health: We’ll help you understand your total health picture, set goals and access additional services as needed.
  • Education: We hope to empower you with the knowledge and understanding you need to manage your condition and minimize health complications.
  • Remote monitoring: We’ll provide you with equipment to monitor your weight, blood pressure and more in real time – all key factors for managing your heart failure.

Please note, our Heart Failure Program does not replace a primary care doctor or cardiologist but acts as an added support to help fill the gaps and provide patients with information and access to care when needed.

How to Get Started

Ask your primary care provider or cardiologist for a referral to our TMH Physician Partners, Services by Southern Medical Group – Heart Failure clinic.

If you’re hospitalized for your condition, your attending physician and our Heart Failure Navigator will also help you obtain a referral. Please discuss this with your healthcare team to schedule an appointment for after your discharge.
